This is an exciting opportunity for an experienced and dedicated Care Manager to join our team at Westmont Community Care.
You will be responsible for the holistic case management of Westmont Home Care Package client needs, goals and preferences.
You will work collaboratively with our team, clients and their families/carers to ensure clients are at the centre of every discussion, ensuring individually tailored care plans, referrals and services are coordinated, records are current and budgets are carefully managed.
- Demonstrated ability to work autonomously with a high degree of initiative
- Demonstrated ability to work effectively with diverse people
- High level computer literacy and client management systems
- Excellent communication skills, both written and verbal
SCHCDSI Award 2010 – Level 3 to 5 depending on experience, with generous salary packaging
