Westmont Aged Care Services Admission and Discharge Policy is based around our ability to provide suitable quality care to meet our resident’s changing needs.
It is the responsibility of the Director of Care and Clinical Services to review the resident’s individual needs taking into account assessments and reports provided by relevant health professionals. In some circumstances we may need to seek other professional advice and assessment to assist in this regard. The following criteria are used in assessing the suitability of a resident for admission or discharge to or from Residential Aged Care.
These factors are considered as part of the assessment process:
- Needs of existing residents
- Staffing levels
- Staff skills
- Building layout
- Specific care needs of the prospective resident
- Clinical nursing skills and/or judgment that may be required to meet prospective resident care needs.
- Availability and accessibility to allied health services
- Behaviours of concern – aggression, screaming, shouting/inappropriate sexual behaviours/psycho geriatric needs.
