Commonwealth Home Support Program (CHSP)

Westmont Community Care is a proud local provider of Government funded Support at Home programs. We can provide services for those through the Commonwealth Home Support Program (CHSP). If you are not sure if you have funding or how to apply, we are happy to advise you on the process to access these programs.

Accessing Aged Care In The Community

Our amazing Community Care team are caring, dedicated and committed to offering you assistance and guidance at all stages of your home care journey.

As your local provider we can offer:

We will work with you to develop a tailored care plan that will support your health and wellness goals. This may include:

    • Identifying and helping you access assistive devices and equipment
    • In home and community exercise or activity programs
    • Strategies to prevent falls
    • Helping you to manage and prompt you in taking medication
    • Working with you to manage your health conditions
    • Other services to assist you to stay at home, such as:
      • Cleaning, garden maintenance, shopping, meals and social support

          Registering with My Aged Care and applying for an assessment

          Unless already registered with My Aged Care, you will need to be registered first before accessing Commonwealth Home Support Program services and apply for an assessment. 

          To access packages or funding, the first step is to register with My Aged Care by calling them on 1800 200 422 or online at My Aged Care Online ServicesMy Aged Care is run by the Federal Government to help with accessing aged care assistance in the home.

          You can self-refer or have a representative (family, friend or advocate) to register on your behalf but you will need to be present during the registration.

          If language hearing or speech is a concern, you can use these free services:

          • Telephone Interpreting Service (TIS) to assist you if English is not your preferred language
          • If you have a hearing or speech impairment, the National Relay Service (mobile and internet charges will apply).
          • If having difficulty accessing My Aged Care or need further assistance, speak to The Older Persons Advocacy Network (OPAN) for help. They can give you information about your rights when dealing with My Aged Care and other aged care queries. Advocates are available to all Australians seeking or receiving aged care services. Call the National Aged Care Advocacy Line on 1800 700 600 (freecall) or contact them online.

          Once you are ready to register with My Aged Care and apply for an assessment have the following ready, to complete the process:

          • Your medicare card which will be linked to your account
          • An understanding of your needs or medical conditions and difficulties you are having at home
          • An idea of services you think you may need or check the Home Care Services available through Westmont Community Care
          • Decide on who will be your representatives who can deal with My Aged Care on your behalf, and have them there during the call or have their details available, as it will be recorded on your account

                Commonwealth Home Support Program Assessment Process

                Once the registration is complete, and you have been assessed as eligible to receive services, My Aged Care will send through a referral to an Assessment Agency.

                The Assessment Agency will contact you within 2-6 weeks to arrange a time for the assessment, which can occcur either over the phone or at home.

                You can have family, friends or other advocates to help you understand the assessment process. For further help see Prepare for your assessment.

                Once the assessment has been completed, you will receive a support plan with your referral codes for each service. These referral codes can either be sent out to all providers in your local area or provided directly to your preferred provider or other organisations such as local health services.

                        Extra support at home

                        Next step - Support at Home

                        If you feel you are needing extra services, and can no longer receive further services under CHSP, you may be eligible for Support at Home. The process to apply for Support at Home is similar to accessing CHSP.

                          We have recently updated fees for our services under the Commonwealth Home Support Program from 1st July 2026 to 30 June 2027.
