We can provide individual support to help you access and feel safe in the community. It can be to take you shopping, to a doctor’s or other medical appointment.
Individual Support may include:
- Pick you up from home and escort you to an appointment
- Essential shopping
- Attending a medical appointment with you and taking notes to help you remember or provide feedback to your family, carer or care manager
